1. To get your account started, complete our Membership Application & Account Card and submit it to us either by mail or by taking it to a branch office.
2. If you are sponsoring a family member or a person who resides in your household for membership in University Credit Union, a Certification of Sponsorship Form must be completed and signed with the sponsoring member's information. Please include this form with the Membership Application & Account Card.
3. If you are not a U.S. person, as identified in the Membership Card under TIN Certification, click through to the IRS Web site to print a copy of the W-8 BEN form and the W-8 BEN instructions. Complete the form and attach it to your membership application.
4. Upon completion of the necessary forms, please bring or mail them and your opening deposit to University Credit Union as directed on the form. If you have any questions or any of the links are broken, please feel free to contact us with any questions.
