Direct Deposit Service

Print this form if your employer, other company or organization doesn’t have a Direct Deposit request form. This form contains the necessary information they will need to automatically deposit funds into your University Credit Union (UCU) Checking or Savings Account. Just complete and submit it to them.

For deposits to checking, you may want to attach a voided check or deposit slip to the form.


This form requires the use of Adobe Acrobat Reader. If you don't already have the Adobe Acrobat Reader plug-in installed, simply follow the button below to get it. There will be instructions on Adobe's web site for installing the free tool, which enables your browser to view documents intended for print through the convenience of your computer.
