  • Notification of Disputed ATM Transaction - If you’ve experienced an unauthorized ATM transaction on your account and need to dispute it, please fill out this form.

  • Cardholder Dispute Statement - Use this form if you have experienced an unauthorized charge or are unable to resolve a disputed card transaction with a merchant on your UCU Debit Card or Credit Card.
  • Mastercard Authorized User - Authorizing another user for your Mastercard can be done easily by filling out this form.
  • Mastercard Automated Payment Service - If you would like to set up automatic payments from your Mastercard at a minimum, specified, or remainder amount, please fill out this form.
